Electronic Medical Record is coming to Lower Columbia Women's Clinic
Lower Columbia Women's Clinic has gone digital. We are excited to embrace technology by transferring our patient charts to an electronic medical record program called Digichart.
The future of health care calls for a nationwide access to your medical record, so no matter where
you travel, your physician will be able to access your health record and make appropriate medical
decisions. The staff at LCWC are excited to be part of this change that will benefit us all.
How does Digichart work?
Digichart is an electronic chart whereby your physician can now access your health record from
the office, the hospital or from home. It is all accessed on line, through a very secure website.
There are firewalls at LCWC and also at Digichart. Any information sent over the internet is sent
encrypted so all your information is secure at all times. The only people who can access your
information are the staff who have been given secure sign ins and passwords. Your account is
monitored for security reasons at all times. All government healthcare privacy regulations are met to
ensure privacy of your medical information.
What do I need to do?
When you first make your appointment you will be given a log in and password. We require that
you log onto Digichart and enter your confidential medical history prior to your appointment. When
you come in for your visit we will be checking your ID and taking your photo, this will help avoid
identity fraud. Once in the room the nurse will then review the information you entered on line and
clarify any questions. The doctor will also review your history. This thorough history enables your
healthcare providers to more effectively discuss your current health and healthcare concerns.
We also will have the ability to send your prescriptions directly to your pharmacy, and receive them
directly into your chart electronically. Your doctor can show you images of your health condition
and print out patient education information custom to your needs.
Where do I go?
Connect to the internet and go to: https://patients.digichart.com
At the patient portal sign in prompt, enter your username and password given to you by LCWC staff.
When you first sign in you will be prompted to change your sign in information. We encourage
that you write your new username and password down after you change it for future portal updates.
Please follow the on screen instructions. The logins you create here will be what you use from now
on to sign in to the patient portal. Please note that we will only use your email address for support-
related communication. We are not set up at this time for confidential email communication.
If you are interrupted while entering your information, you can return to it at your convenience and
pick up where you left off. Typically, it takes 30 minutes to complete this process. If you are a
returning patient, you will only need to update any changes in your medical history.
You may complete your questionnaire as early as 2 weeks prior to your scheduled appointment,
but no later than 2 days prior to your appointment. If for some reason you are unable to complete
your medical history on line, please call our office no later than 2 days prior as we may need to
reschedule your visit .
